A:

Unfortunately, it seems that no current product can accurately record and transcribe a meeting.  A leading voice recognition program, Dragon NaturallySpeaking, can do a good job with one voice, but not with a group of people.  See:

Apparently automatic transcription of meetings is still a subject of scientific research:
