Do i really have any rights resident or not

i am a resident of costa rica, libre condicional the last 9 years after being a pensionato the first 7. i was paryalized in cima in 2005. despite having made a number of agreements with cima, do i have any rights when the hospital changes the rules on me monthly with cima knowing  (bills which have been paid and is being paid about 1/2 million to date) is the only hospital i can be treated at? even with the hospital knowing i must go to mexico or argentina in latin america, the U.S., canada, or a handful of countries in europe and that missing one treatment needed apox. every 3 weeks is life taking, let alone life threatening and the bills for( everytime they have been sent to the DEPT. of Veterns Affairs under the U.S. Veterns FEDERAL MEDICAL PROGRAM) have  been  paid
