Not sure what program you want approval for. If it is for free cell phone the phone must be in the name of the person applying. Most all programs no matter what they are have the same requirement. Therefore, you need to transfer the phone out of your deceased husbands name into your name (if you are the one applying). If your daughter is the only one that qualifies the phone must be in her name.
