Sould the police have made sure Deborah Jeane Palfrey (the D.C. Madam) would be accompanied at all times? She always said she would rather die than face charges, and now she's committed suicide.
Deborah Jeane Palfrey was not in custody at the time of her suicide. Most people who are not in custody would object to 24-hour monitoring. In retrospect, perhaps someone should have seen the signs of imminent suicide and petitioned for some kind of involuntary protective custody. But even Palfrey's mother stated that she had no indication that Palfrey was suicidal. So I don't think that the authorities had a basis for continuous monitoring.
