Alaska Airlines Flight 261, a McDonnell Douglas MD-83 aircraft, crashed on January 31, 2000 in the Pacific Ocean about 2.7 miles (4.3 km) north of Anacapa Island, California. The two pilots, three cabin crewmembers, and 83 passengers on board were killed, and the airplane was destroyed. Alaska 261 was a scheduled international passenger flight from Lic. Gustavo Díaz Ordaz International Airport (PVR), Puerto Vallarta, Mexico, to Seattle-Tacoma International Airport (SEA), with an intermediate stop planned at San Francisco International Airport (SFO). The subsequent investigation by the National Transportation Safety Board determined that inadequate maintenance led to excessive wear and catastrophic failure of a critical flight control system during flight. The probable cause was stated to be "a loss of airplane pitch control resulting from the in-flight failure of the horizontal stabilizer trim system jackscrew assemblys acme nut threads. The thread failure was caused by excessive wear resulting from Alaska Airlines insufficient lubrication of the jackscrew assembly. Japan Airlines flight 123 disaster 1985

Japan Airlines Flight 123 was a Japan Airlines domestic flight from Tokyo International Airport (Haneda) to Osaka International Airport (Itami). The Boeing 747-SR46 that made this route, registered JA8119, crashed into the ridge of Mount Takamagahara in Ueno, Gunma Prefecture, 100 kilometers from Tokyo, on Monday August 12, 1985. The crash site was on Osutaka Ridge (おすたかのおね, Osutaka-no-One?), near Mount Osutaka. All 15 crew members and 505 out of 509 passengers died, resulting in a total of 520 deaths. It remains the deadliest single-aircraft accident, the second-deadliest aviation accident after the Tenerife disaster in history, only counting victims on board.

Compass Challenge IX - Paro Bhutan

In February 2003 a Boeing 737-700 Succesfully completed 11 test flights at Paro International Airport, Bhutan. THe series of flights included 2 technical demonstration flights and 8 Customer relations flights with Druk Air, the national airline of Bhutan. The 737-700 demonstrated engine-out takeoff procedures, single engine missed approach and go around procedures, and landing procedures for landing on both directions on the runway at Paro, utilizing a reduction to idle thrust of one engine to simulate a failed engine. Analysis of the flight data recorders showed that the aircraft met or exceeded performance expectations. Several performance options were subsequently studied to improve available takeoff weight at Paro, including Accelerating the airplane to a speed 10 knots faster than the minimum takeoff speed for improved climb performance, and additional optomization of the turn procedures. A new thrust bump rating, the CFM56-7B26/B2 would offer at least 2% more thrust than the CFM56-7B26 used on the actual demonstration aircraft. This improvement would have allowed Druk Air to fly a 737-700 with full passenger load to all of it's planned initial destinations. This video was inspired by this test flight... Although the Boeing 737-700 proved itself worthy and able to fly to Paro safely and efficently, Bhutan's Government decided to purchase the slightly larger A319 from Airbus. (from wiki) Airbus signed a memorandum of understanding with Drukair in July 2003 for two 114-seat[32] Airbus A319-115, powered by two CFM56-5B engines,[33] for delivery in the second half of 2004.[34] The purchase of the two jets, valued at Nu. 3,534.36 million,[14] was the biggest single purchase ever made by Bhutan, and was largely responsible for a 250 percent increase in the Bhutanese trade deficit over the previous year for the financial year 2004 2005.[35][36] The Bhutanese government issued Nu. 1,767.18 million in government bonds to pay for one aircraft[14] and for only the second time in Bhutan's history intended to seek a commercial loan for the other aircraft,[16] however, in October 2004 it announced it would instead seek a soft loan for this purpose.[14] The first Airbus A319 arrived in Bhutan on 19 October 2004;[32] the date chosen after a Buddhist astrologist was consulted to ensure the aircraft arrived in Bhutan on an auspicious day in the Buddhist calendar.[14] Before entering service on commercial flights on 31 October 2004,[43] Drukair took their A319 on a country-wide flight in honour of the ascencion of Crown Prince Jigme Khesar Namgyel Wangchuck to the Chhoetse Penlop.[14] The second aircraft was delivered by Airbus to Drukair in December 2004.[44]
