I am having a problem i regularly make mistake ...

I am having a  problem i regularly make mistake which i dont intend to, at work or with friends, many times i feels why i did it or that? and try to avoid it but still I repeat them, Because of which i am getting demoralized and confused, I unable to understand why this happening with me? Do i need a medical help?

No, I think you react fast and say what comes to mind first. Try to stop and think it out and see if your remark can be taken wrong.  If it is just say "Im sorry, I shouldnt of said that"

Improving Mental Concentration, Better Mental Focus, and Stronger Willpower!

Kashif, My Friend--

     Most likely your issue is a simple matter of failure to control your attention in the present moment - a problem of not being fully aware of where you are, what you are doing, what you are thinking, and what you are feeling, all simultaneously.

     You see, a person who develops the power of concentration seldom (if ever) makes blundering, embarrassing, self-sabotaging mistakes because their controlled attention keeps them in charge of body, mind, and behavior on a consistent basis.  People who have a strong power of concentration do not suffer from irrationality, clumsiness, reactionary explosiveness, mindless talking, or thoughtless behavior.  You don't want to be a victim of any of these things, therefore you should consider training your mind in order to develop your own power of concentration.

     Do so, and you will benefit from the increased self-confidence, self-control, self-discipline, improved personal productivity, expanding intelligence, and consistently uplifting emotional states that accompany mental mastery.

     It will change you.  It will change the way people treat you.  It will change your life.

     Therefore, I recommend that you get started ASAP.
