Applications are handled by your county's Social Services dept.  Whether you qualify or not will depend on your income, assets, and family size.  You will need to have documention to prove your income and assets - pay stubs, bank statements, etc.  Call the social services dept. to ask what the criteria is for your county and to make an appointment to see a case worker.  Good luck.
