The above costs will be fixed and will vary by contractor. If you do the work, then you can get a price for materials from any box store. Lines 1 and 2 are eliminated if you are installing over concrete.
The variable will be the price of tile. The price varies widely.
This may not be a terribly satisfying answer, but it describes what you are looking at.

Vinyl starts under $1/sq ft
ceramic starts at $4/sq ft for flooring
Stone $10/sq ft if you're not being picky.
plus the cost of preparing the room to accept flooring (what flooring is there presently?)
installation starts at $20 for the cost of buying the tools and printing out step-by step to lay the peel-and-stick vinyl tiles over an old linoleum floor in good condition up to $5/sq ft for 'standard' installs.
