How do handicapped people travel on a plane ? What if a paralized person needs to go to the bathroom on a long flight?

There are many accessible planes out there . The improvement in the airline companies is very slow, before a handicapped person travels he need to ask a lot of questions about the whole procedure of the flight. Who is going to board him? From where etc.

About the toilet it is not that simple I found this article that says:

"If disabled passengers find themselves on one of the smaller planes that does not have an accessible lavatory, they may be required by the airline to be catheterized."
