I have gone 7 months wiyhout a carfare check due to a medicaid mistake in address

medicaid.com       i went away for 4 months for a domestic issue from jan.1-may first. the first day out ,i went back to my program(staten is.univ hosp multi service center to get back to my meetings and to be properly medicated)before i went to rik.isl.  i had carfare accounts before.so this is the problem. somehow all my medicaidmail has been going to this other addressand i have not received not 1 carfare check since may 1 when i left r.isl.so far ive been down to medicaid 4 times atleast filling out address changesand what not .this i dont understandbecause i am mentally disabled on soc.sec.dis. w/my address as 209 ave i bklyn ny 11230 bsmt.apt. also my food stamps go to my address of  209 the previous.this is 7 months can you please help me!
