What does "frequency" mean on an IEP

how can a student with an IEP be considered "overserviced" in an inclusion classroom.  Also, on the IEP when you see "5" under frequency, does this refer to the amount of days per week the student is in the classroom or is it the amount of classes per day; or hours per day? This is in the middle school setting where students change classes for math, social studies, language arts, science, spanish and AIS?
