Go to the Facebook administration or contact page.  They should have a place you can email them and ask them to take your page off.

We are in the process of deleting all employee Facebook accounts that have e-mail accounts tied to the company .  The information below was supplied to our corporate attorney's it explains the difference between deleting and deactivating your account .  Hope this helps .

"Deactivating your account isn't the same as deleting it since you can re-activate your Facebook account simply by logging in again .  You log in one time and your back on with a fully activated functioning account .  This means that your profile and other personal details are still lurking on Facebook's servers . You will need to contact Facebook directly to actually delete your account .  There is a deletion link on your account .  You must NOT try to log in once the request is made , as it will cancel the deletion request or the deactivating request . If the account becomes active again it is due to the account holder logging in and continuing to use the site .  Moral of the story if you want off Facebook stay off it is that simple .
