I'm guessing she is seeing a counselor of some sorts. As much as people would blame the counselor, it's actually the client's (Britney's) responsibility to work hard and get her feet on the ground. It doesn't appear that that's what Britney wants. Because if it was, she wouldn't be frequenting the bars and clubs or stripping into her underwear to jump in the water in the wee hours of the morning while the paparrazi snaps pictures. Also, a counselor can be blue in the face telling her things that will help her get moving in the right direction and it can be worth nothing if the client isn't really listening. In this case, Britney clearly has no person whom she wants help from, so any counselor's efforts will be fruitless.
