Something doesn't add up

I just don't understand.  20 years ago, those who were medically indigent went to the county hospital.  Others qualified under Medicaid.  What has become of these programs?  Did too many doctors refuse to accept Medicaid payments?

Also, if the average cost to treat each person was valued at $350 (as per the report), why are people who can buy cigarettes and eat at McDonald's unable to pay this?  My kids didn't eat at McDonalds until they were in their teens.  We were poor and I found I could cook and serve several days worth of healthy meals for what McDonald's charged for one dinnertime unhealthy meal for each family member.  This just doesn't add up.
