It may depend on your state. Most states define a complete medical record as ALL information pertaining the patient. That inlcudes letters and reports from other physicians and ncillary services as well as financial records (billing). If a patient requests a release of his or her records. You must provide all your medical reocrds, notes, labs, etc. You are not required to release records from other physicians unless specified in the release request, or if in your judgement, such nformation is important to that patient's care. Psychiatric records and HIV results require separate release forms. Financial records can only be released by a legal subpoena.
