Hi,

   I'm an RN in a hospital and doctors are not supposed to write something in your chart that makes him seem not to like you,  I have not heard of anything like that.  They usually write some vital signs , the diagnosis, the treatment and the nursing care.

   If you're bothered by this, talk to that doctor and let him know why you think he does not like you.  If you don't get any satisfactory answer, then take the chart to the CEO of the hospital and show it to him.
