Workplace abuse

What organization do I go through to file a formal compaint about the following? 1. Supervisor harassment causing emotional stress and having to take LOA. 2. Supervisor failing to report medical injury and issues causing further damage and an extended LOA which resulted in loss of job. 3. When complaints were made about Supervisor, many workplace problems began. No changes in Supervisor behavior were made and stress escalated.
