Figured it out finally. Turns out it was due to some sort of corruption in Outlook itself. Running PST Scan multiple times indicated that there is a problem, but couldn't fix it.

Eventually I moved a large number of mail items (~10,000) from the default PST to an archive PST, and that fixed the issue.

Oh well.
