Unfortunately, as we have seen recently, contamination is a reality in the modern commercial food chain.  For example, with the spinach fatalities, the spinach was washed according to the company's quality standards, but the water was contaminated by field water containing pig feces.  It never occurred to them to check their water supply for impurities.
