Underground Electrical Problem

We have underground electrical power supplied by a utility and the "ground" or "neutral" failed causing high voltage and low amperage which affected various motors and lighting.  Repairs were made by the utility company necessitating digging a 25' long trench 4' deep and 5' wide.  After a 2 month period the "hot" wire failed and the trench was reopened and the fault repaired.  Workers for the power company indicated the second incident was probably caused by nicking the insulation of the "hot" line during the initial excavation or during backfilling by a rock due to not using proper backfill materials to protect the line.    What are your thoughts?

sounds right. Underground feeders need to be installed properly. I always put them in conduit for that very reason.
