Trust bank account switched to another bank in name of trust: how do I update schedule B, Separate property?

I switched money in a bank account listed as separate property on schedule B in our family trust to another bank with the living trust as beneficiary. What do I need to do to show the money moved to the new account is still meant to be separate property to go to a minor child, as per the trust? Do I need to add the new bank on Schedule B and remove the original bank account that is now closed?  Any formalities?
