Trafalgar Square and The Tate Modern

My parents are very fit, 70 year old culture vultures planning a holiday in London. They are planning to visit Trafalgar Square and the National Portrait Gallery, they also plan to visit Tate Modern, is it possible to walk between the two locations and if so would you mind giving some simple directions.

The National Portrait Gallery is on Trafalgar Square and Tate Modern is South of the river - it is walkable but it is best that they have a map of where they are going.

I walked it when I was 21 and very fit and it was tiring but invigorating.  70 seems pushing it to me.   The tube takes you to Pimlico and you walk from there...
