If you take the sperm and the egg from the two intending to be parents and put it in a surrogate. What blood type would the baby have? Would it be a combination of just the mother and the father or would the surrogate play a role in the blood type?

There is some sort of problem where antibodies, directed against the Rh factor, can pass from an Rh negative mother to the Rh positive unborn child and cause all sorts of misery. This can be treated with Rhogam. This will not change the blood type of the unborn.
