I am not from Ohio. Call the powers that be in your state who regulate this. They will tell you what to do. In the state I am licensed in we are required to keep up our licence no matter what (ie...even during recovery periods).

The license is pretty important proof that you become an LPN well the procedure will just be like the one you take on getting your first license but this time it would be more easier

To be a licensed practical nurse , you must finish a vocational college or other approved program for licensed practical nurses. You need registration in all provinces and territories and must complete the Canadian Practical Nurse Registration Examination in all provinces and territories.
