It has been a few weeks since we spoke- thanks for contacting me-
I wish to proceed with the application for asssitance thru AZ health
system. I wish to begin immediately with all the necessary paper work
and forms - especially for finding REHAB/Asst.Living/Long-term care
due to Husband Ralph: Heart problem, ALZ + PD..82 yrs. and gonna
be evident something will appear to need the above... Plus for the
past 3 months I've become very sick- today Diagnostic Mammo-
Wed. released by Heart Hosp.@ BB Hosp. from ER entry Sun. to
in-pt. care 4th floor of Telemetry... dozens of test, scans, etc. to
determine why Elevated BP, swollen legs, hands, extremely ex-
hausted -Today is Step #2- the Mammo. See family Dr. Mon.
Results & Blood Tests avail. at 854-5000 (heart hosp.) punch
MED RESULTS to request my records of the stay this week.
Ongoing Dr. visits are the monthly CKs with Colon Dr. Cho for
elevated Liver function -?? bowel since Colonoscopy/Endo done
in may '09 - I will be prepared for his doing 2nd colonoscopy since
my bowels are no bigger than a Pencil size and only 4in. @ time.
I'm truly sick - and unable to function for more than 5mins @ time.
Anything you can do to give direction/Help is very much appreciated
