Physical custody already ordered to foster parent in Tn.but now son resides with me. How can i get back custody of my son in Ma.?

Back in 1999 i was incarcerated and the lady that had me said she'd watch the kids til i came out which was a month for hindering apprehension. Anyway this lady was 14 when she had me and gave me up for adoption which means she is not their grandmom! She had to go to foster parenting classes. I do not know how she received permanant custody of my children. I was under the impression it was temporary till i got my stuff together. However i have fought this loady for 10 yrs trying to get my kids back. I went to TN. where my kids are and my son came back with me to Ma. However she had given me a letter stating she has legal and pemanant custody over my son, But gave me permission to enroll him in school, to get him a psycologist because he has ADHD and be resposible for him. With this paper i can't enroll him in school because she has custody? Anyway i have been 3 yrs clean, went to U-MASS college and became a Certified Alcohol and Drug Abuse Counselor. I have a full time job and my own apartment. My son doesnt want to return to Tn. How can i get custody or temporary custody so i can take care of him. This document was notorized in Tn. but it isn't a court document nor was it drawn up by a lawyer so is it even legal/? My son is living with me in my apartment some one please help me i have been fighting 10 yrs now he's finally with me and nothing is working out for the best interest of my son. God has brought me this far but i keep getting the run around! the court order for custody was from New Jersey
