Open ground means that a third wire, the ground (usually bare or green wire) is not connected.  It used to be that extra grounds were not installed, and many older homes have two wire electrical systems in all or part of the house.  You may have an open ground because someone put a three prong outlet where where a two prong one should have gone, or for other reasons.

Open Ground is an unconnected ground and is usually associated with a cable that is delivering a radio frequency signal. When the ground shielding that signal is open or unconnected a symptom might be unwanted interference with the television channel that is being viewed. This occurs when there is another station broadcasting on the same frequency as the cable channel that is tuned in.  Ideally the ground connection surrounds the signal wire inside the cable which prevents the interference.
