Motion to Appeal the denial decision of my CSS class membership by the Special Master

My city where I live is Los Angeles. I am a "Catholic Social Service "(CSS) class member. I recently appealed before  a designated Special Master the Service (US.CIS) class membership denial decison which I applied along with my legalization application I-687. The Special Master (SM) based on the Rule 53(g) of the Federal Rules of Civil Procedure (FRCP) just ordered a decision pertained to my appeal which was served to me on July 9, 2008.

While I do not agree with Special Master decision and given the fact that I under the FRCP I am being awarded 20-days to appeal the Master's decision, I would like to know  three things: (1) what kind of motion do I submit to appeal the SM decision, (2) where in Los Angeles do I submit this motion to appeal, is it at a Civil Court?, and (3) what would be the fee that I have to send with this appeal?  I appreciate your prompt response.
