Does Bluetooth usage drain cellphone  battery?

Sure - your cell is consistently sending and (trying to) recieving data.

it is best to turn off bluetooth when not in use.

Oh yes it does, that is why some of the best bluetooth accessories now will go to sleep when not in use to save battery life.
