Maximum POP that can be used for company group medical insurance Premium and Health FSA list

I was told that the maximum percentage that an employee can pay of employer's group medical insurance premium for the employee through a POP (Premium Only Payment) is 50%. Is that correct? If not, can the POP pay 100%? Do you have somewhere I can reference this answer? Also, do you have a good current list of items that can be deducted through a Health Flexible Spending Account?
