What is the best way to lose weight and keep the weight off?

Cut the sugar. These days, sugar is added to almost every edible product, so you have to learn to read the nutritional value labels. Do not even consider anything that has more than 15% sugar. Give up all sodas, snacks, chocolate etc.
Do not eat ANYTHING after 6 pm. 5 pm is even better.
If you are going to eat something "forbidden", eat it early in the morning.
Excercise. But it has to be something you enjoy, or it will be a torture for you. The simpliest, safest and very effective way to excercise is... to walk. It needs to be fast-paced and last at least an hour, though.
Do not lie to yourself. Never try to excuse yourself with "Yes, it is very sweet but has lots of minerals and fibre". Say "I failed today" instead.

Exercise is the only true, long-term way to control your weight. Start off with lots of walking and a bit of weightlifting. If you exercise enough, eventually the weight will come off.
And skip any fad diets and just try to eat natural foods - fruits, vegetables, lean meat, etc.

First speak to your physician to determine if you can diet, then go to a dietician to develop a program specifically for you. Eat three meals a day and nothing between meals. Exercise is also important especially push-aways (from the table to counter any temptation. Don't be discouraged and give it all up; stick to the program religiously and you should be feeling the results within a month.

Eat a little less and move a little more than you normally do on a consistent basis and you will lose weight and keep it off.
Eat a balanced diet and do some form of exercise every day.

Drink plenty of water/green tea.
Get into a healthy eating routine. Eat steam food, vegetables and fruits.
Get into a regular exercise regime with strength and cardio. If at the gym, ask for a personal trainer.
