Leaky window

I replaced a pane of a bay window about 1 1/2 years ago along with several others throughout the house.  The one on the bay window is again clouding up is now leaking.  The installer keeps re caulking but the problem is only getting worse.  The one next to it is starting to show signs of clouding now.  Any ideas? I believe it is still under warranty.
