According to spinner.com, Ella Fitzgerald blanked on 'Mack the Knife': "Oh, what's the next chorus/To this song now?," she sang. Ella laughed it off, and it won her a Grammy.
I think it's more embarrassing what Fergie did peeing in her pants on stage during a performance in San Diego.

I happen to think forgetting the lyrics is not nearly as bad as falling off the stage, like Beyonce did in her Orlando concert.
Or Britney Spears falling on stage at her concert in Japan.
