We also have a Bichon, and she get will get hot spots.  I use neosporin on the spot and rub it in.  It seems to help.  The vet okayed this treatment and also suggested that I give her half of a benadryl tablet in the am and half in the pm.  This takes a couple of days to work, but it does.  It takes at least a ten day period to see the spot healing.
