Always make sure you follow the recipe and measure ingredients properly. Using bread flour, not all-purpose flour, is important, as bread flour has more protein and will hold the structure of the bread better. Check the date on your yeast container to make sure your yeast is not old. Make sure you are using the correct recipe size for the size of your cooker. If the recipe only makes a 1-pound loaf and your machine has a 2-pound capacity, the loaf will look small. Also, make sure you are using the proper settings on your machine for the recipe.

gravity pulls the dough down and compacts the bread into a hard loaf.

sounds like the yeast was dead. maybe it was dead when you put it in the machine. proof some to check. maybe another ingredient (the liquid) or the machine was too hot. liquids should be comfortable for your finger; if you pull your finger out because it's burning, the heat will also kill yeast.  if the machine is heating during the mixing process, it is defective. [the dough naturally warms through yeast fermentation.] there is a small chance that you put the yeast and the salt together and the contact killed the yeast. that's why many recipes instruct one to mix the salt into the flour.  hope this helps
