Fulton County Board of Commissioners has denied my request.

My reason is that the commissioners are members of the board which is a single entity.

Simplified: "The Board has denied..."

To figure out the number of the subject, it is sometimes helpful to remove all modifiers and objective phrases. Both "Fulton County" and "of Comissioners" modifies the core noun, which is "Board." The subject is singular so the verb form is "has" instead of "have."

The Board is the one that denied. Which one? Fulton Count Board of Commisioners. Therefore the correct  way is has denied...

You want to make sure that the subject of the sentence agrees with the verb. The subject of a sentence is never in a prepositional phrase; "of Commisioners" is a prepositional phrase, so eliminate it. The subject is Board and since it is singular, you need a singular verb - in this case, has. The Board has denied my request.
