it could be several things you did not mention the type of filter you have. is the filter large enough for the size tank? in order for the charcoal to work it needs water flowing through it therefore it must go into the filter. need to read the filter instructions to find where the filter bag goeshow often are you doing water changes. goldfish and koi are known to be "dirty" fish meaning they poop a lot.
