You need to write to the doctor, hospital, laboratory, or other source requesting a copy of all medical records for the purpose of further medical treatment. Be sure to say it is for further medical treatment. If you do not, they may charge you "copying and labor fees" which are sometimes very high. Most states have laws that prohibit these kinds of charges if the records are for the above. I believe that the law in many states only requires medical providers to keep records for six years, so it is always good to get copies of your records as often and as soon as possible after they are made (xrays, blood tests, etc.). Some doctors, labs, hospitals, etc. have their own request forms which they require you to sign. You might call and find out before writing your letter.
