What is the best way to create a telephone list that reflects addresses, cell phone numbers, home phone numbers, e-mail addresses by category such as doctors, friends, plumbers, electricians, etc and that is free and easy to do ?

Open Outlook and navigate to the Contacts pane. Click "View" and select "Arrange By" from the drop-down menu.
