As far as having a valid British visa in an expired passport, you can use it with your new passport as is the case in USA. But I don't have exact knowledge for the other countries mentioned. This is I am telling you on my personal experience as when I was issued a 5 years Multiple entries British visa, I was also handed over an information page by the British High Commission in Pakistan which contained that particular information...
