Choosing a room mate

I'm looking for a room mate to share my apartment, but I want to make sure I pick the right one.

What should the application include, and how do I screen?

This really depends on you. You may be a person that likes quiet and then you'll look for someone who doesn't seem too wild. You may be a very clean person and then that'll become an important factor.

In general I would try to find someone who you think you'll have good chemistry with, someone you'll enjoy spending time with, someone who seems flexible and has good communicating skills.

These are the things I think are most important but as I said, It's really up to you.

I remember once being with a roommate which at first I despised  an we turned out to be best friends. I can't really tell you how to screen them , I guess like mentioned tidiness is important but you never know how can you tell a person if he is tidy? Try and see if he left another apartment before joining you and ask him why did he leave there. I guess in these situations it's about a pure gut feeling.
