CRCF's in South Carolina

S C Dept Health Human Services Div of Facility Services  Where can I get a list of all CRCF's for senior citizens and the disabled in S C? I need them all from Assisted Living to Adult Day Care. I need the list to detail complete contact info [ address, phone, web, etc]. Lastly I need to know they are funded [ federal, state or local gov. or if paid for by the user or by other sources ].

I recently went to a new doctor and noticed he was located in something called the Professional Building. I felt better right away.
