Eat right? Hard to really know if that is correct as I do not know what yr diet consists of. Maybe there is a problem with certain items on the diet itself
Cardio - I need to know at what percentage of your maximum heart right you train at and for how long. Do you breathe heavily while doing cardio? Do you sweat? Can you talk normally while doing so?
Weights - Same here. A question of intensity. Not sure what yr regimen is
