Well, if it's the simplest program that you're looking for, and you'reusing Windows, you should look at the Paint program that ships with Microsoft Windows.

You will find it under the Accessories menu.

To add text to an image, open the image, and click the "A" icon on the toolbar on the left.

The disadvantage of Paint though is that it will always place the text in a colored rectangle, which may not be what you want.

What Yaniv said about using paint is mostly true. Paint (At least in Windows XP) does not always place the text in a coloured rectangle.

Paint DOES however place a background behind the text. Simply select a colour for the background (From the colour palette at the bottom of the screen) and right-click the desired colour.
This will ensure the background around the text (the coloured rectangle) is the desired colour.

To remove the background colour completely, you simply need to choose white as the background colour, and then click on the lower of the collection of three shapes icon on the left of the screen.

This will effectively make the background colour clear/non-existant.

Clicking the icon as shown above can also be useful on images if you wish them to be placed closer together without a horrible white border obscuring the other image.

Well what could be simpleror better than this...FotoTagger... it is FREE FREE FREE but it is great to add text or annotate and even put multiple annotations on the pic, for instance to name parts of an item. It does it the way you want. I use it quite regular and am extremely happy with it.

Here is the link to the site that even has a forum etc on it for the program and its users     http://fototagger.com/

Happy texting
