I recently became aware of a 401K Plan that I have ...

I recently became aware of a 401K Plan that I have with a former employer of 19 years ago.  The phone number I've been able to ascertain is disconnected, and I'm not sure if the address I have is acurate.  How can Ir retreive the money I have in this 401K?
